Endometriosis was found in 80% of women suffering from The Chronic pelvic Pain Syndrome. However, in our research, we discovered that Interstitial Cystitis, a Painful Inflammatory condition of the Bladder was also present in more than 80% of women with Chronic Pelvic Pain syndrome and the two painful diseases (THE EVIL TWINS) are associated with each other. We were able to make early diagnosis of the two painful diseases and treated the patients early in their diseases state with good results.

Post Hysterectomy chronic pelvic pain:
High incidence (80%) of interstitial cystitis in pelvic pain patients after hysterectomy. The bladder should be evaluated routinely before hysterectomy. It may avoid hysterectomy if the cause of the pain is bladder origin.

This study showed high incidence of insterstitial cystitis in patients with Post hysterectomy pelvic pain with ovarian residual syndromes or remnant syndrome, therefore the bladder should be evaluated before removing the ovaries.
over 70% of these pelvic pain patients were diagnosed with I.C. in this study.

The overlapping syndromes; Interstitial Cystitis and Overactive Bladder. Symptomatology of IC and OAB are very similar, OAB patients should be evaluated for interstitial cystitis. The study found close to 60 % of OAB patients have IC.

One theory of IC is that the bladder is "leaky," which can allow irritating potassium to penetrate into the bladder wall and provoke symptoms. If that’s true, said these researchers, the levels of potassium in IC patients’ urine is likely to be low, and it should go up when treatment with oral heparinoid-type therapy is successful. The researchers measured not only potassium but also sodium and creatinine in spot urine samples from new, symptomatic, untreated patients with IC who met all the NIDDK IC diagnostic criteria for research, in samples from patients who said they had a 50 percent or greater symptom improvement after four or more months of treatment, and in control patients. The 37 new patients had significantly lower levels of potassium relative to creatinine than the 18 control patients. The 50 successfully treated patients had significantly higher levels of potassium relative to creatinine than the new patients. The investigators also measured potassium over 24 hours, which also showed that 30 new patients had lower average potassium levels and lower potassium levels relative to creatinine than 47 controls. The researchers concluded that the higher levels of potassium with successful treatment reflects repair of the bladder mucosa, making the bladder less "leaky" and allowing less potassium to leak into the bladder wall.

     The clinical incidence of Pudendal Neuralgia in this study suggests this disease entity and the "Evil Twins" (Endometriosis & Painful Bladder Syndrome) should be at the top of the differential diagnosis for chronic pelvic pain syndrome as the "Evil Triplets."
